Abstract
:
Background: Secondary caries is one of the most common complications of existing esthetic restorations, such as composite resin. Silver Diamine Fluoride (SDF) has been recently introduced as an interim therapy to manage caries. Aim: To assess the impact of SDF in halting the progression of secondary caries in extracted human primary molars in comparison to fluoride varnish or no treatment. Material & Methods: This was an in-vitro study. Cervical cavities were prepared on sound surfaces of 30 extracted primary molars, followed by composite restorations. After that, the demineralization of dentin adjacent to the restorations was initiated. Later, the sample was divided randomly into three equal groups; group 1 received SDF. Group 2 received 2.5% Sodium Fluoride Varnish with Hydroxyapatite. In group 3, no treatment was performed (control group). Secondary caries progression was assessed by one blinded and trained examiner through using Micro-computed tomography (micro-CT) to evaluate the area change of the lesion. Results: There was statistically significant difference in the rate of change of demineralization areas between the three groups (p=0.034). The SDF group showed consistently smaller measurements of secondary caries lesion area (-27099.53 ± 43499.26 µm2), followed by 2.5% fluoride varnish with hydroxyapatites (-13134.53 ± 62648.71 µm2). The area of secondary caries lesions formed in the control group was the largest in comparison to other groups (28259.19 ± 49000.09 µm2). Conclusion: Silver Diamine Fluoride is more effective in arresting and preventing secondary caries progression in primary molars in comparison to fluoride varnish or no treatment in vitro.
